  • Separation agreements, divorce paper, or support obligations filed within one year.
  • Six months of your bank statements and all canceled checks.
  • Six months of paystubs to determine your income.
  • Tax returns filed for the last two years.
  • Copies of your retirement, IRA, or 401k statements.
  • Download and complete the forms HERE.
